Hopefully the truth comes out one day
Detective Amoral, who was in charge of the original investigation, believes Madeleine died in the apartment, probably accidentally, and that her parents either disposed of the body, or know who did hide the body.
I believe the same, but we'll never know for sure

Kate and Gerry McCann took him to court over the book, and initially they won, and the book was banned, but eventually, on appeal, they lost, and the book became available again

So that's about as clear as what happened to the little girl. So much time has passed that it seems we will never get to the bottom of it. Personally I hope she reappears someday and she is unharmed
